Requesting a Review:
A number of forms which may be used for procedures under the Acts are provided in the Freedom of Information and Protection of Privacy Regulations. Although these forms are not required in order to request access or a review, you may find them very convenient:

Form 3: Consent to Disclosure of Information
Form 4: Consent to use of Personal Information
Form 7: Request for Review (Applicant)
Form 8: Request for Review (Third-Party)
