Breach Notification pursuant to section 70(2) of PHIA from Custodians
Once the Custodian makes the decision not to notify an individual, pursuant to section 70, the Custodian must notify the Review Officer as soon as possible by: 
 
1. Calling immediately; and 
2. Completing and sending this email within 48 hours of the phone call. 
 
The Custodian will receive an email acknowledging receipt of the Breach Notification from the Review Officer advising that no further action will be taken by the Review Officer at this time.  Should the Review Officer decide to initiate her own motion privacy investigation the Custodian will receive notice under separate cover.
 

Notification of disclosure of personal health information for research pursuant to section 57 of PHIA from Custodians
Once the custodian decides to disclose personal health information without the consent of the subject individual to a researcher, pursuant to section 57(d) the Custodian must notify the Review Officer.

Notification must be made within 48 hours of disclosing the personal health information by completing and sending this email.

The Custodian will receive an email acknowledging receipt of the research disclosure notification from the Review Office advising that information will be recorded for statistics and annual reporting purposes.  Should the Review Officer decide to initiate her own motion privacy investigation the Custodian will receive notice under separate cover.
