Notification of disclosure of personal health information for research pursuant to section 57 of PHIA from Custodians

Once the custodian decides to disclose personal health information without the consent of the subject individual to a researcher, pursuant to section 57(d) the Custodian must notify the Review Officer (Information and Privacy Commissioner).

Notification must be made within 48 hours of disclosing the personal health information by completing and sending this email.

The Custodian will receive an email acknowledging receipt of the research disclosure notification from the OIPC advising that information will be recorded for statistics and annual reporting purposes.  Should the Commissioner decide to initiate her own motion privacy investigation the Custodian will receive notice under separate cover.
